Kindergarten Registration for all Habersham County Elementary Schools will be held at Demorest Elementary (GA Highway 365 and Demorest-Mt. Airy Highway) March 14-17, 2016.
Children must be five years old on or before September 1, 2016 to enroll. Parents are encouraged to bring their children on the assigned date for the school district in which they reside. However, parents may bring their children to any of the following registrations:
- Monday, March 14, 2015 Cornelia 4– 7 p.m.
- Tuesday, March 15, 2015 Baldwin and Clarkesville 4– 7 p.m.
- Wednesday, March 16, 2015 Demorest and Level Grove 4– 7 p.m.
- Thursday, March 17, 2015 Fairview, Hazel Grove, Woodville 4– 7 p.m.
(NOTE: Please wait until 4 p.m. to arrive for registration to ensure that Demorest Elementary buses/students have left the campus. Be sure to allow 2 hours for the complete registration process.)
You will need to bring the following:
1. Habersham County Schools Enrollment Form available on the Habersham County School System website at www.habershamschools.com or at all elementary schools.
2. The child’s original birth certificate with the Seal, File Number, and Signature of the Registrar (copies cannot be accepted.)
3. Georgia Certificate of Immunization – Form 3231. Please bring your child’s current immunization Form 3231. Habersham County Health Department personnel will not be available during registrations.
4. The Child’s Social Security Card
5. Proof of Current 911 Address (Current utility bill: Power bill, water bill, natural gas bill, cable/satellite bill with the physical 911 address listed.)
If you do not have proof of current 911 in your name, you must:
a) Complete the Statement of Legal Residence Form w/Notarized Signatures (this form is available on the Habersham County School System website at www.habershamschools.com or at all elementary schools)
AND
b) Attach the current proof of 911 in the owner/lessor name listed on the Statement of Legal Residence Form
6. Certificate of Eye, Ear, Dental and Nutrition Examinations – Form 3300 (REV 2013). There are two ways to obtain this form:
A) Children may be given an eye, ear, dental and nutrition screening during registration. Form 3300 (REV 2013) will then be completed for their files.
OR
B) If your child has already been given these examinations at the Habersham County Health Department or at their physician’s office, you may bring the completed Form 3300 (REV2013) to registration.
School personnel will administer an assessment during registration. It is very important that you bring your child for this process so that plans can be made for your child’s entrance into kindergarten. Be sure to allow 2 hours for the complete registration process. Parents will accompany their children throughout registration.
