Northeast Georgia Medical Center (NGMC) Habersham has been honored with a prestigious award for its various community-focused initiatives, according to county officials.
The Georgia Hospital Association (GHA) Patient Safety and Quality Award recognizes exceptional achievements in healthcare quality and patient safety, and Habersham’s hospital is now named as the latest recipient.
The hospital also received the esteemed Circle of Excellence Award. To be eligible for this distinction, a hospital must earn the Patient Safety and Quality Award four times within a five-year period. NGMC Habersham is one of six hospitals to have achieved this remarkable recognition.
NGMC Habersham earned the Circle of Excellence Award through its collaboration with the Georgia Department of Public Health and the Habersham County Emergency Services Community Paramedicine Program – a partnership that has significantly advanced post-stroke care in the region, according to county officials.
In closing gaps in healthcare and social services, the initiative sees that that patients receive comprehensive, in-home follow-up care after hospitalization – including support for chronic disease management and education on healthy lifestyle choices.
The Habersham County Emergency Services Community Paramedicine Program plays a major role in the well-being of local residents, particularly those in underserved areas where healthcare access is difficult.
Through the program, community paramedics build trust and relationships with patients so that individuals become more comfortable when seeking care. The program also offers routine health assessments, helps manage chronic conditions, reduces unnecessary hospital visits and connects patients to vital resources while navigating the healthcare system.
